capX Student / Early Practitioner Enrollment Form
Before starting the enrollment form, please note the following:
- Review the form in advance. Ensure you have all necessary documents ready before beginning.
- The form cannot be saved mid-process. You must complete and submit it in one session.
- Donation: A $10 donation to the Foundation is requested to complete your submission. Click here to make your donation. Upon donating, you’ll receive a passcode to submit your form. To streamline the process, consider making your donation first and obtaining your passcode before starting the form.
